The Prostatic Portal Practitioner Training Program Waiver ................. It is important to share that it is not advised to massage a prostate that suffers from inflammation, cancer and other illnesses. We advise to ask your models some questions about their sexual health in general, urination routines, sexual dysfunction, chronolical illness and use medication before inviting them to be your model. I understand that it is my responsibility to make sure that the model I will be using for this training has a healthy prostate. If any doubt, the model will consult with a physician prior to and regarding his participation in this TPP training. I understand that the training is meant for experienced bodyworkers and that working with prostate massage and healing can potentially release trauma responses. I assume full responsibility for any risks, injuries or damages, known or unknown, which I might afflict on my prostate model. We will guide you to the best of our ability to guide you to work with a prostate in a safe protocol. I will not hold Stine Krage and Jasper van Ruth responsible for any injuries or actions arising out of, or in any way connected to, the training with my or my models participation. I understand that an informed consent requires; the model participating to be fully informed and fully understanding of the treatment or intervention. The participant must give their consent for participation and consent to follow all processes/guidelines prior to engaging. The privacy and confidentiality of yours, and your models, personal information is a high priority. The following is a statement of understanding and an informed consent. I understand that information shared within the group will remain confidential information to any person/facilitator/assistants/employer without my informed, voluntary, and written or verbal consent. I understand that all group members/facilitators are bound by this confidentiality. Confidential means that no information, using people’s names/identification, is shared outside the group. The technical definition of confidentiality in a therapeutic context is “The ethical principle or legal right that a physician/therapist/psychologist/social worker/counsellor or other health professional will hold secret all information relating to a patient or client, unless the patient or client gives consent permitting disclosure. I understand that The Prostatic Portal collects personal information of the participants and their models in order to provide supervision to the participants. This information is retained, protected and not shared outside of course requirements, nor without consent. Use of Course Materials Agreement Participants are welcome to use the contents of this course in their own work with clients or students, with appropriate attribution of source, including some written material. I agree not to use, copy or repackage any of this material for distribution to others or for sale, without written permission from The Prostatic Portal. *
